The acronym R.E.A.C.H. stands for Rental Emergency Assistance for Community Housing, symbolizing our commitment to reaching out to those in need, housing them, and ensuring they receive the care and support they deserve. We serve as a financial solution, bridging the gap until long-term assistance such as Section 8 can cover the remaining costs.
Our programs to assist our communities
ALWP (Assisted Living Waiver Program)
Through our medical-based program, we cover 90% of assisted living rates, enabling individuals with medical needs to access the care they require while maintaining their independence.
Community Support
We advocate for individuals with substance use disorders and mental health challenges, providing essential support services such as housing navigation, assistance with doctor appointments, and housing deposits. Our goal is to ensure a smooth transition from institutional settings to community living, filling the gaps in mental health treatment and support.
Enhanced Care Management
Within the comfort of their homes, we offer comprehensive case management services including caregiving, respite care, and tailored meal plans catered to Medicare and MediCal recipients.
